'If it ain't broke, don't fix it'

In any self organising system we can get locked into a local equilibrium. Any deviation from this seems inferior - so we make no such deviation. But in order to reach a more global equilibrium there is a need to re-think almost everything. What are the mechanisms, institutes and structure for doing this? We are complacently happy with many concepts and procedures which could be greatly improved. How is the effort to improve them going to be made? "If it ain't broke, don't fix it" would be less dangerous if it did not seem so logical.
